@corycalendinemd: The ACL surgery where we do not replace the ACL. Most torn ACLs need a new graft. A tibial spine avulsion fracture is different. In children roughly 8-14 years old, and occasionally in adults, the ligament is stronger than the still-developing bone it anchors into. When the knee twists or hyperextends, the ACL holds and pulls a fragment of bone off the top of the tibia instead. The ligament is intact - the anchor is not. These injuries are graded by displacement. Nondisplaced fractures, and partially displaced fractures that reduce well, can often be managed with immobilization in extension. Completely displaced fragments, or those that will not reduce, typically need surgical fixation. A common reason a fragment will not reduce is soft tissue trapped in the fracture bed, most often the medial meniscus or the intermeniscal ligament. That is why MRI matters in planning, both to assess the fragment and to identify associated meniscal, cartilage, or ligament injuries. Arthroscopically, the fracture bed is cleared of clot and debris. High-strength sutures are passed around the base of the ACL, shuttled down through small tunnels in the tibia, and tensioned against the bone to seat the fragment back in its anatomic position. Suture and screw fixation are both options, and the choice depends on fragment size, comminution, and protection of the growth plate in younger patients. #TibialSpineFracture #KneeSurgery Recovery matters just as much as the repair. Stiffness is a known complication after this injury, so stable fixation and early, guided motion are central to getting full extension back. 👇 SHARE your #ACLinjury experience & insights . . 🎥 @JBJSmedia @Arthrex - Tibial Spine Avulsion Fracture Reduction Using ACL Repair TightRope® Implant
