@the.legal.np: She came in with a broken ankle. 21 years old. Hurt doing farm work. It should have been a straightforward admission. Her family's lawsuit alleges she was given multiple pain medications, wasn't checked on, and was found too late. Here's the clinical point every nurse needs to hear: sedation comes before respiratory depression. A patient who is "resting comfortably" after stacked opioids may not be resting. She may be drifting toward apnea, too sedated to call for help. This is why we: 🔹 Assess sedation level before every opioid dose, not just pain scores 🔹 Reassess when doses are combined or stacked 🔹 Push for continuous monitoring (pulse ox, capnography) for high-risk patients 🔹 Chart what we saw, when we saw it, and what we did about it In court, the question won't be "Was she comfortable?" It will be "Who was watching her breathe?" These are allegations, and the case hasn't been decided. The lesson for practice stands anyway. Rest in peace, Dale. 🤍 #legalno #medicalmalpractice #legalnurse #nursestiktok #medsurg

Why did a pharmacist approve both opioids to be given in whatever frequency without dc’ing one or calling the doctor to clarify.

Why, as a nurse would you be giving both narcotics??? I would question that big time and most likely would hold one!

I literally wouldn’t allow IV narcotics if I wasn’t on a continuous SpO2 monitor

I’m finishing up pre-reqs right now so I’m at the tip of the iceberg but I feel like any patient in the hospital should be monitored at all times? Keeping up with vitals and such can literally save a PT’s life. This is a clear example of that

I presume if she was relatively healthy and just had a broken ankle she was on a med surg floor - what hospital allows IV opioids on a med surg floor?!

And even if u didn’t know to cancel the order why aren’t we getting a set of vitals before each dose. Especially to people do don’t take them on a regular. Surely her respiratory status decreased overnight

Accurate documentation, clear endorsements, and open team communication are essential to keeping everyone aligned. If a new medication is indicated for a patient without a verified written order, I will immediately contact the physician for clarification.

Phone orders not allowed. They all have access to EPIC and must place their own orders in.

Before administration of another dose, you're supposed to do an assessment of respirations and O2 to safely administer

In the emergency room we take verbal orders all the time, but have to reassess the patients once a medication is given.
