There is a rocker arm recall for that problem. Worn rocker arm screws are caused by early Gen 5 DD 15 engines didn't have an oil rifle in the exhaust rocker arms causing them to run dry at idle. Excessive idle is the usual cause of this failure. You can see idle percentages if you run a DDEC report. Updated rocker arms are available for that engine.

I highly recommend updating to the drilled exhaust rockers to avoid the adjuster screw failure
