1) Always assume it’s loaded. 2) always keep the weapon pointed in a safe direction 3) NEVER put your finger in the trigger guard until ready to discharge to weapon. If these very simple rules were followed we would not be seeing this.

Proper gun knowledge, proper gun safety you are never supposed to attempt to clean a gun that is loaded. That is the first step to cleaning a weapon. Is you make sure of it it’s not loaded and that there’s nothing in the chamber so with that being said the story seems very fishy to me.
