Yellow Card reporting: practical answers for patients The hardest part can be opening the form and wondering what belongs in it. Here is how to prepare a clear account, find the right reporting route and understand what the service can actually do for you. Do I need an NHS login to report a side effect? No. You can submit a Yellow Card as a guest without signing in. NHS login is an alternative way to create or access an account, not a requirement for reporting. Guest reports do not automatically appear in your account. Keep your report reference and confirmation so you can identify the report if you need help later. Can I report a side effect already listed in the leaflet? Yes. A known effect can still be worth reporting, particularly if it is serious. The MHRA asks for serious suspected reactions to established medicines even when they are already recognised. Read the leaflet for advice about your symptoms as well. Reporting and getting help with the symptom are separate steps; one does not replace the other. Can I report a problem with an over-the-counter or herbal medicine? Yes. The scheme covers more than prescription tablets. You can report suspected side effects involving medicines bought without a prescription, vaccines and herbal products. Include the actual product name where you know it, rather than just writing 'a supplement' or 'a painkiller'. The reporting service also has a route for products that are not on its list. What information helps a Yellow Card report? Useful details include the product name, dose, treatment dates, when symptoms began, what happened afterwards and other medicines being taken. Relevant medical history can also help assessment. Describe what you remember accurately; distinguish an approximate date from an exact one. A short timeline can make the account clearer without asking you to decide what caused the problem. What happens after I send a Yellow Card? You receive confirmation, and you may be contacted for more information. Depending on the issue, the MHRA, a manufacturer or a medical specialist may investigate. A report is recorded even if it does not lead to an individual investigation. Sending the form does not promise a personal diagnosis, a medicine change or a particular regulatory action. If you are unsure about a medicine name, have the box or dispensing label beside you. Your pharmacist can help identify it. Keep the details for the healthcare conversation as well as the report. You do not have to write like a clinician: a clear description of your own experience is useful. Separate what you noticed from what you suspect. For example, describe the timing and how the symptom affected you rather than presenting a definite diagnosis. That leaves the safety team with an honest account to assess, while your healthcare team considers what support or treatment you need. A report about a suspected quality problem has a different focus from a report about a symptom. Describe what seemed wrong with the product and identify it as clearly as you can. The app's extra reporting options do not mean that every unusual-looking pack is fake; ask a pharmacist for help rather than deciding from appearance alone. Yellow Card cannot give medical advice. If symptoms worry you, ask your pharmacist or prescriber what to do and follow the medicine leaflet's urgent instructions. Do not change prescribed treatment on your own. Sudden throat or tongue swelling or severe breathing difficulty needs 999; do not wait to finish a report. What would make completing a medicine-safety report easier for you? Please keep personal medical details private. Raj Patel, UK pharmacist. General education. MHRA guidance checked 28 October 2026. https://yellowcard.mhra.gov.uk/ #YellowCard #Mhra #SideEffects #PatientSafety #pharmacist
