I would argue that being as the creature states you mill a card per 1 damage delta that way that each mill is a separate trigger thus making the water crystal add four to each. so 5 cards per 1 life.

something I've always wondered about is things like sphinx's tutelage with "mill more" effects, when does the mill more kick in? The first time they mill something? So every instance of tutelage they'd mill 6? Or after they've stopped milling more? And does sphinx's tutelage's ability to make you mill more apply to all 6 cards?

“For each” is a common phrase used to indicate a quantity of an event relative to something else. It doesn’t mean to perform the action that many times unless specified like drawing. Gaea’s cradle is one mana making event not one event for each creature.
