@emswami: Be cautions intubating patients w/ severe metabolic acidosis + compensatory respiratory alkalosis -Hyperperpnea drives CO2 down supporting the pH -Removing respiratory drive will drop pH -> HD collapse -Mech vent challenging too as must mimic preintubation respiratory parameters -Instead, reach for NIV to support work of breathing #emergencymedicine #emergency #ems #paramedic #emergencynursing #criticalcare #resuscitation #dka #intubation #airway

Insulin Iv and strap on a bipap with 1:1 RN before she gets tired. In a few hours I bet she would turn around and prevent intubation. We only allow bipap on ams if in direct icu care

when I had metabolic acidosis I remember breathing like that totally involuntarily. crazy sensation. Our brains are so cool.

the analogy I always use with my students is you don't cover a bad smell with air freshner... you get rid of the source of the stink!!! treat the cause not just the symptoms.

Why go through all that? For insurance ? That’s why all doctors are getting so rich? What’s the correct intervention for DKA or HHNS?

But we can adjust the ventilator parameters to mimic the body’s response to a severe acidosis, if you don’t intubate that patient is gonna get tired and respiratory failure would be imminent.

If we were to increase the RR after intubation and give some sodium bicarb. Would that sufficiently compensare?

If your concern is to mimic pre-intubation respiratory parameters- then set your RR high and VT to 8 mls/kg

I mean you could still intubate as long as you give the patient a high enough minute ventilation

Everything is fine :
Match rate ventilation if you have to tube. Monitor abgs and etco2 adjusting as needed. fluids, insulin drip, monitor lytes especially k. Serial glucose and lytes. Close the gap.

Let me ask a question when you say NIV do you mean BiPAP also is the purpose of using BiPAP to avoid respiratory failure due to muscle fatigue? Since the patient is not having a respiratory issue, I assume that is what you’re using it for?
