@dr.garthdavis_01: If you think about it, we evolved hundreds of thousands of years ago and we evolved in a time of feast or famine. It was advantageous for us to evolve a very large stomach, so when food was around, we would be able to eat a large portion to sustain us when food was not around. Additionally, the stomach secretes, a hormone called grhelin, which strongly increases hunger. This makes sense when food is not around but over the past hundred years we now put our bodies into a situation where food is constantly around and we're constantly driven to eat. In the single anastamosis duodenal switch, or SADI, we remove part of the stomach so that capacity is smaller and there is less grehlin hormone so hunger drops significantly. We then divide the duodenum and attach it to the ileum. This createsa situation where food hits the incretin secreting cells sooner thereby stimulating GLP1, the same peptide hormone used in Wegovy/0zempic. Additionally, by bypassing the upper intestine we have have less surface area for food absorption. The results have been spectacular. 10 year weight loss of 35%, Wegovy has 19% at 3 years. Interestingly, we are seeing very low complications. Recent 4 yr study showed no vitamin deficiencies at 4 years. Less dumping and ulcers than the bypass, and less reflux than the sleeve. This surgery is only appropriate for people with real morbid obesity with BMI over 40, and lifestyle changes are still essential! #weightloss
