The 0-10 pain scale is one of the most commonly used tools in medicine. The problem is that acute pain and chronic pain don’t always fit into the same framework. Someone with acute pain may rate a 7/10 because they’re experiencing a sudden injury or illness that completely disrupts their day. Someone living with chronic pain may also rate their pain a 7/10, yet still be working, taking care of their family, or making it through the day because they’ve spent months or years adapting to it. That doesn’t mean they’re suffering less. It means they’ve learned how to function despite it. This is why pain scores should never be interpreted in isolation. The number matters, but understanding how pain affects someone’s ability to work, sleep, exercise, socialize, and enjoy life often tells us far more than the number itself. As a pain physician, one of my most important questions isn’t just “How bad is your pain?” It’s “What is your pain preventing you from doing?” That’s often where the real story begins.
