Hashimoto’s isn’t simply a “thyroid problem.” It’s an autoimmune disease that targets the thyroid. That distinction matters because it changes how we think about what is actually happening in the body. In Hashimoto’s thyroiditis, the immune system produces an immune response against thyroid tissue. Over time, this can damage the thyroid and reduce its ability to produce adequate thyroid hormones. Eventually, that can lead to hypothyroidism. But hypothyroidism and Hashimoto’s are not the same thing. Hashimoto’s is the autoimmune disease. Hypothyroidism can be one of its consequences. And this is where thyroid pathology becomes particularly important. Someone can have Hashimoto’s while their thyroid hormone levels and TSH are still within the laboratory reference range, particularly earlier in the disease process. This is why I don’t want to look at TSH in isolation. Depending on the individual case, thyroid investigations may include TSH, free T4, free T3 and thyroid antibodies, particularly thyroid peroxidase antibodies, or TPOAb, and thyroglobulin antibodies, or TgAb. Then I want to understand the bigger clinical picture. What does their iron status look like? Are they getting adequate selenium? What is their vitamin D status? Is coeliac disease relevant? Are there gastrointestinal issues or nutrient deficiencies that need investigating? What else is happening metabolically, hormonally or clinically that could be contributing to how this person feels? This doesn’t mean thyroid medication isn’t important. For someone who needs thyroid hormone replacement, medication can be essential. But replacing thyroid hormone and understanding the autoimmune disease occurring alongside it are two different conversations. And if someone tells me, “My TSH was normal, so I can’t have Hashimoto’s,” I don’t consider that enough information to make that conclusion. This is exactly why understanding the pathology matters.
