As a psychologist, I’m horrified by what the Lindsay Clancy case appears to reveal about gaps in our mental healthcare system. There are three critical problems I keep coming back to: 1. The medications. Lindsay was prescribed numerous psychiatric medications, with multiple changes over a relatively short period of time. Who was overseeing the entire medication picture? Who was tracking her response to each change, watching for worsening symptoms, and making sure every provider knew what the others were prescribing? 2. The follow-up. Just 19 days after leaving McLean Hospital, Lindsay killed her three children. For someone experiencing severe and rapidly changing psychiatric symptoms, what monitoring and follow-up occurred during those 19 days? Was the intensity of that follow-up adequate for her level of risk? 3. The fragmented care. Lindsay reportedly interacted with multiple doctors, prescribers, therapists, emergency services, and a psychiatric hospital. When care becomes fragmented, critical information can fall through the cracks. Someone needs to see the whole clinical picture and coordinate the treatment plan. None of this excuses what happened to Cora, Dawson, and Callan. Three children lost their lives, and their father suffered an unimaginable loss. But we should be able to hold two truths at once: what happened was horrific, and we should examine whether serious failures in our mental healthcare system contributed to the trajectory that preceded it. If we want to prevent another tragedy, those questions matter. #LindsayClancy
