It’s because that scene was set shortly after the surgery so her body might still be in shock and recovering but at this point her memories haven’t actually started to come back yet so the “I am Julian” part is just a representation of the two of them restoring their relationship slowly and as time goes on and her body heals enough, she’ll start getting memories back
