When I found out my premature baby had his umbilical cord clamped immediately in a US hospital (I missed the birth) I was very concerned. In contrast, China recommends delayed cord clamping until after cord pulsation stops (5-10 mins, cord is white and all the placental blood is now in the baby)—but US hospitals still don’t wait until the cord is completely finished pulsing. Why wait? The remaining placental blood contains oxygen, iron, stem cells and immune cells that support baby’s transition after birth. (Exceptions are made for infectious disease and emergencies.) For premature babies, delayed cord clamping has also been associated with lower mortality, fewer severe brain bleeds and less need for transfusions. So if you hear, “We’ll delay for one minute,” you can absolutely ask push back and request optimal cord clamping until the cord is white, floppy and no longer pulsing.
