Record when it started, what you observed, the patient’s vital signs, oxygen therapy administered as prescribed, actions taken, who was informed and the patient’s response. As a nurse a nurse. Write in a way that any person reading the report will understand. As long the way you wrote gives clear message of intervention. Some the high grade in English escapes our brains when we're tired and busy. The most important thing is you intervened, called for help and recorde in a simple English that is can be understood by everyone. The most important is what you did when patient's condition changed and was urgent.
