Imagine being prescribed testosterone by your healthcare provider… and then being told at the pharmacy, “We don’t dispense testosterone to women.” Thank you @drrachelrubin for bringing it to the FDA! This is a conversation we need to have. There is currently no FDA-approved testosterone formulation specifically indicated for women in the United States. FDA-approved testosterone products are labeled for men, so testosterone treatment in women is prescribed off-label.  But “off-label” does not mean illegal, experimental, or automatically inappropriate. The FDA itself explains that healthcare professionals generally may prescribe an approved drug for an unapproved use when they determine it is medically appropriate.  And we DO have clinical evidence. International guidelines support carefully dosed transdermal testosterone for appropriately evaluated postmenopausal women with hypoactive sexual desire disorder (HSDD). When a female formulation isn’t available, guidelines describe using approximately one-tenth of a standard male dose, with monitoring to keep levels within the physiologic premenopausal female range.  Yet women can still encounter barriers when they try to fill these prescriptions. And this month, the FDA itself convened an entire public workshop on testosterone use in menopausal women, acknowledging both its growing off-label use and the need for better evidence, long-term safety data, female-specific dosing research, and potential drug development.  That’s the part that frustrates me. Women shouldn’t have to navigate a system where there is no FDA-approved female testosterone product, clinicians have limited female-specific formulations available, and some patients then encounter another hurdle at the pharmacy. Women deserve better research. Better education. Female-specific treatment options. And evidence-based access when treatment is appropriate.
