IBS, fibromyalgia, migraines, chronic pain conditions, and anxiety are typically treated by separate specialists with different diagnoses. Recent research is identifying mast cell activation as a shared underlying mechanism across all of them. A 2024 study by Van Remoortel et al. in Cell Reports Medicine reviewed mast cell modulation as a therapeutic target for IBS pain, showing mast cells directly drive pain-sensing nerve activation in the gut. Research by Theoharides and colleagues has documented thalamic mast cell involvement in fibromyalgia pain sensitization. A 2025 systematic review found IBS and fibromyalgia share mast cells, pro-inflammatory cytokines, and gut-brain disruption as common mechanisms. The migraine connection comes through CGRP release from mast cells, which is the same target as newer migraine medications. These conditions are now sometimes grouped together as nociplastic disorders. The mast cell connection across these conditions is well-supported in mechanistic research but mast cell stabilizers aren’t yet first-line treatments for IBS, fibromyalgia, or migraine. Treatment decisions should be made with a clinician who understands your specific symptoms. I’m not a doctor, just sharing research I find interesting. Sources: Van Remoortel et al. (2024).
